The original cover was drawn by Ross Andru and Sal Buscema, but Marie Severin was called on to greatly revise the cover, retaining most of the background elements, as discovered by Nick Caputo on Comic Art Fans page:

Also, in the back of the comic there's a page recounting the basic info of Kull. The text reads in part "...the cover...is the combined effort of Ross Andru, Sal Buscema and Marie Severin."
