Originally distributed, as part of a limited edition in a "special protective cover" with a certificate of authenticity, only as a mail-in offer through "Wizard: the Guide to Comics" magazine #64.

Although there is no cover price, the actual cost was unknown (United States) ; Canadian orders were 4.00 USD and 'other foreign orders' were 6.00 USD - payable in US funds only. Offer expired January 15, 1997.
